Glass Vase ID Needed

Heavy vase - 9" tall with flanges to sides in clear glass also tears of glass stemming from bottom up - fades from deep green to clear at bottom - flat polished base

Is it Czech?

Czech would be my first guess too. I'm not familiar with this model of vase, though.
