i'm thinking Polish, quite modern (just an uninformed opinion) - LSA type.

It has a pontil mark so hand blown
I assume most LSA glass is factory mould blown isn't it?

No. Much LSA glass is made in Krosno and that is hand made in the main. Factory hand made, but hand made.
